OverviewMedical Records SpecialistFullTime, 80 Hours Per Pay Period, Day ShiftPosition SummaryCompiles and maintains statistics for the department and other areas. Maintains integrity of Master Patient Index. Provides support to staff positions during peak workloads and fills in vacations and holidays. Analyzes/monitors reports. Assists with quality improvement activities and special projects. Maintains electronic filing. Processes all orders, monitors receipt of signed orders in medical record. Assists with training of new employees. Serves as a telephone backup in the department.   • Compiles and maintains statistics for the department and other areas.
  • Serves as a support to clerical and analyst positions within the department.
  • Assists in the training of new employees.
  • Maintains medical record attachments, including signed forms, wound images and images.
  • Assists with quality improvement activities and special projects.
  • Maintains MPI integrity by performing merges, additions, deletions, and revisions.
  • Enhances professional growth and development through participation in educational programs, current literature, in-services and workshops.
  • Attends meetings and serves on committees or teams as required.
  • Performs other related duties as assigned or requested.
  • Demonstrates ability to meet or exceed departmental quality and quantity standards.
  • Adheres to hospital and departmental policies and procedures, including those related to safety and infection control.
  • Prepare patient assessment packets, including admission, recertification and post hospital.
  • Process signed and unsigned Orders and 485s.
  • Follow up on the Order Tracking Report weekly and according to the Order Procedure. Reprocess unsigned orders at end of episode as task appears on action screen.
  • Fax the discharge / transfer summary / episode detail summary and Case Conference / 60 Day Summary to the physician as task appears on action screen.
  • Copy documentation and assist with ADR preparation.
  • Update HCHB and computer programs as appropriate.
  • Support billing by verifying paper notes, if needed.
  • Perform workflow for Medical Record Specialist in HomeCare Home Base.
  • Run OASIS/HIS validation reports. Transmit data each Monday by 4:00 p.m. to CMS.
    • Run OASIS/HIS Roster Report monthly along with Oasis C Reports and distribute copies to each branch.
    • Complete OASIS workflow in HomeCare Home Base upon acceptance of OASIS/HIS by CMS.
    • Correct errors on Roster Report and route to the Clinical Manager for review and submission to the Quality Coordinator with the QI data on the 4th of each month.
QualificationsMinimum Education:None specified; will accept any combination of formal education and/or prior work experience sufficient to demonstrate possession of the knowledge, skill and ability needed to perform the essential tasks of the job, typically such as would be equivalent to a high school diploma or GED. Preference may be given to individuals possessing a HS diploma or GED.Minimum ExperienceTwo (2) years experience in an acute care Medical Record Department preferred. Detail-oriented, good organization skills, ability to work independently, strong interpersonal skills.Licensure RequirementNone
