  • Ensure understanding of Hospital’s Community Medicine policies
  • Area mapping, household and beneficiary registration
  • Prepare monthly calendar of activities/events.
  • Home visits to assess health status
  • Deliver appropriate information , provide counselling and support to local communities to access hospital facilities and treatment
  • Register patients who need clinical support as per the hospital policies.
  • Maintaining data of all designated households, individual beneficiary
  • Coordinating with the Hospital Administration for the beneficiary’s visit to the hospital
  • Mobilizing stakeholders for the community events conducted by the Hospital viz., immunization camps, community workshops, etc
  • Report medical outcome for treated patients to Superiors.
  • Liaisoning with Government Authorities if required.
  • Fluency in Tamil / Telugu must
