  • Accurately validate and abstract Hierarchical Coding Condition (HCC) diagnosis codes from outpatient/Inpatient provider documentation prior to billing.
  • Analyses medical and clinical diagnoses of chronic conditions, procedures, injuries into ICD-10 Coding
  • Good knowledge on Risk Adjustment related general guidelines.
  • Knowledge of medical terminology, anatomy, physiology, disease processes, and pharmacology.
  • Abstract other information as applicable to the project from electronic and handwritten medical records in accordance with established CMS and client-specific guidelines
  • Requires a strong knowledge of ICD-10-CM diagnostic coding and HCC documentation requirements.
  • Work as part of a team and achieve the team quality and productivity standards.
  • Identify the errors in clinical documentation at a facility and physician specific levels and document the same
